Welcome to the Special Needs Web page. In this section you will find several forms that will allow you to "Help Us, Help You". This program is designed for you, the community member, who may have some special care needs that are different than that of another patient. By helping us help you this will allow our members to be aware of the care that is needed and furthermore it will allow us to input a special notation into our computer system signaling us that there is some information that we need to look at before we arrive to take care of you in your time of need. It is our goal to reach out to every member in the community who may have or need special assistance from our department in a time of need. The list of needs could span from a premature infant who will be on an apnea monitor (short term) to that of a child who is going to be on a ventilator (long term). Maybe it will range from a adult patient who doesn't have a caregiver living with them to a obese patient that we will need extra assistance for lifting, to even that of a cardiac patient who has a mechanical heart device that requires a pump that runs on power and there is a power outage. There is no limit to what this program can do to allow you to "Help Us, Help You". Ken Lybarger will be in charge of this program, please feel free to email Ken with questions about this program.
